We are a Medicare-certified agency providing intermittent skilled nursing, therapy services, and personal care to patients in their homes under their physician's plan of care.

All referrals are evaluated by our internal medical team.

SERVICES PROVIDED 

• skilled nursing assessment

• post-surgical care

• wound care

• physical therapy

• speech therapy

• personal care (assistance with bathing)

• recommendations for equipment, home modifications & home safety

 

SERVICE LIMITATIONS

• no services provided outside Sumner County

• no palliative care

• no pediatric care

• no respite care

• no social services 

• no ventilator care

We accept Medicare, Medicaid, Private

Insurance, Veterans, and Private Pay.

Patient eligibility may depend on insurance requirements, including the following:

• Services must be ordered by physician

• Patient must have a qualifying skilled need that can be provided on an intermittent basis for Medicare services,

• Patient must be homebound. (Other insurances vary with this requirement.)
